Licensed Practical Nurse

We are looking for a reliable LPN to care for our residents in a professional and considerate manner. You will work under the supervision of a registered nurse or doctor. Candidates must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding.

ABOUT THE POSITION

Responsibilities:​

  • Assesses residents on admission and at least monthly, as well as when conditions change. Document appropriately.

  • Assess and reassesses pain management as needed and educates the resident and family regarding pain management.

  • Assesses the resident’s condition and nursing needs; sets goals; prescribes appropriate nursing action.

  • Documents changes in resident’s condition and nursing care on progress notes and plan of care.

  • Meets current documentation standards and policies.

  • Notifies physician of changes in resident’s condition and follows through until appropriate action is taken.

  • Notes and carries out physician’s orders in a timely manner.

  • Gives thorough report to oncoming shift.

  • Follows through, as needed, on information given by shift report, resident or family regarding resident concerns.

  • Treats residents and their families with respect and dignity. Identifies and addresses psychological, cultural, ethnic and religious/spiritual needs of residents and their families.

  • Supervises other members of the healthcare team, as needed, to accomplish the plan of care which has been developed.

  • Functions as a resident advocate by protecting the resident’s rights.

  • Administers medications by using the “rights” and performs treatments safely.

  • Interacts with residents, families, co-workers, managers and other departmental personnel in a kind, knowledgeable and friendly manner.

  • Consistently includes resident and family in planning of care, treatment, teaching needs and discharge plans.

  • Functions as a leader for the NAs, CNAs and CMAs, by delegating in a manner that contributes to the excellence of the nursing unit to which LPN is assigned.

  • Follows proper infection control procedures and supervise supportive staff in infection control procedures.

  • Demonstrates ability to stay calm in an emergency and/or stressful situation and gives everyone a feeling of confidence that the situation is under control.

  • Consistently follows policies and procedures of the facility.

  • Must administer medications as ordered by physician and in MARS.
